Guideline for Abnormal Liver Function Tests (LFTs)
A Guide for GPs
This guideline is designed to assist General Practitioners (GPs) in the management of adult patients with abnormal liver function tests (LFTs) who present without symptoms or signs of liver disease. It provides an overview of the initial assessment, appropriate follow-up, and referral criteria to specialist services. This is intended as a guide and should complement, not replace, clinical judgement.
The guideline covers key topics including:
- The scope of the guideline
- Recognising patterns of abnormal LFTs
- Initial assessment steps for patients with elevated LFTs
- Guidance on when to repeat LFTs and when to refer to the Liver Clinic
- Criteria for referral and identifying patients who require specialist care
- Cases that do not require referral, specifically those with Alcohol-related Liver Disease (ArLD) and Metabolic Dysfunction-associated Steatotic Liver Disease (MASLD)
Additionally, the appendices include important resources such as:
- A list of abbreviations
- A guide to non-invasive liver screening
- Pathways for managing alcohol-related liver disease and MASLD
This structured approach aims to streamline the decision-making process for GPs, ensuring timely and appropriate care for patients with abnormal LFTs.

What happens following the receipt of your referral?
Upon receiving your referral, our consultant team assess it, assigning a priority based on clinical need. Subsequently, an appointment date is allocated, taking into account this priority. Please be aware that waiting times can fluctuate depending on the specialty in question.
For urgent priority referrals:
- An appointment date is promptly issued.
- A letter confirming the date, time, and location of the scheduled appointment is sent to both the patient and the GP.
For routine priority referrals:
- The patient and GP are informed of the routine priority.
- An estimated waiting time for the required specialty is provided.
- Confirmed appointment dates are issued 6-8 weeks in advance of the scheduled date.
